Complete Your Intake Forms
You'll receive intake paperwork prior to your first visit. Please complete it before your appointment so we can make the most of your time together.
Prepare for Your Visit
Have your insurance card, photo ID, medication list, and any relevant records ready. For telehealth visits, find a quiet, private space with a good connection.
Show Up as You Are
No need to have it all together. Come with your questions, your concerns, and your whole self. That's exactly what we're here for.
Insurance and Payment
The Pink Practice LLC is a private-pay, direct-care practice. We do not bill insurance directly at this time.
A superbill (itemized receipt with diagnosis and procedure codes) can be provided upon request so you may submit for potential reimbursement through your insurance plan, HSA, or FSA.
We believe transparent, relationship-based care is worth it. Payment is due at the time of service.
We Accept
Superbills are available upon request for out-of-network insurance reimbursement, HSA, and FSA submissions.
